How to Sign in to Your NPDB Report
If you are signing in to view your NPDB report for the first time, you will need information from the notification letter you received from the NPDB.
You will need to enter the Account ID from your letter. If you are viewing a report for an organization, you will enter an Access Code. If you do not have the letter, you must request a Self-Query to access your report.
Effective September 26, 2024, the NPDB requires all accounts to be identity verified to sign in. This is required to comply with the federal government's cybersecurity standards issued by NIST, the National Institute of Standards and Technology. The NPDB has partnered with ID.me to meet these standards.
How to Sign in to Your Report
Note: Only the subject of the report can sign in to their report.
- On the home page, select Sign In to View Your Report.
- On the Sign In To Review Your Report page, select Sign in.
- If you already have an account with ID.me, sign in to your account. Otherwise, select Create an ID.me account.
- Complete ID.me's identity verification process. ID.me will display each required step.
- After you complete identity verification, select the option to allow access to the Department of Health and Human Services' External User Management System (XMS). This is required to connect your NPDB account to your ID.me account.
- On the NPDB Account ID page, enter your Account ID, then select Continue. Your Account ID is on your notification letter.
- If the system displays a password or access code form, enter the code from the notification letter. If you have signed in to your report in the past, enter the password you set for your account.
- If the system displays an Account Information page, review your contact information and update it. Select Submit.
The next time you sign in with ID.me you will go directly to the Your Reports page.
