How to Update Your Account Information for a Report

Use the following steps to update your email address, phone number, and mailing address, or to create a user ID and password. You cannot change any information on the report itself.

To make sure report information is secure, we require practitioners enter their date of birth and Social Security Number (SSN) or Individual Taxpayer Identification Number (ITIN) to access their account information. Please have this information on hand before signing in.

To Update Your Account Information for a Report

  1. On the home page, select Sign In to View Your Report.
  2. On the Sign In page, enter your account ID and password, then select Sign In.
    Note: If you are signing in to your report for the first time, use the report number and password on the notification letter.
  3. On the Practitioner Identification page, enter your date of birth and your Social Security Number (SSN) or Individual Taxpayer Identification Number (ITIN), then select Continue.
  4. On the Your Reports page, select Your Account.
  5. On the Account Information page, you may update your:
    • Email address
    • Phone number
    • Mailing address
    • You can also create a user ID and update your password.
      Note: You do not need a user ID. Once added to the Account Information page, you can sign in to your account using your report number, email address, or mobile phone number.
  6. Complete the Certification section and select Submit to the NPDB.
