What is a personal account?

A personal account creates one central location for health care professionals to access the individual Self-Query and Report Response Service. Complete your NPDB profile to get started.

A free personal account allows you to:

• Search the NPDB for reports that match the information you provide and view the results.
• Respond to an NPDB report if you have one.
• Manage your personal and contact information.
• Get notified by email and text message if any future reports about you are sent to the NPDB

With a $3 annual subscription you can:

• Get your current certified Self-Query response
• Access, download, and save your Self-Query response at any time
• View a list of entities that received your report(s), if any, in a query response
