How do I complete my personal account profile?

To complete the profile in your personal account:

  1. Select Sign In on the top right of the NPDB home page, then select Personal Account.
  2. On the Self-Query/View Your Report Sign In page, select Sign in.
  3. Sign in to your personal account with your ID.me email and password.
  4. Review the information you agreed to share from ID.me. If it is incorrect, please update your ID.me account exit icon.
  5. Select Complete your profile on the personal account home page. For the fastest and most accurate search results, add all the information you know. Be ready to provide:
    • State health care license information (if you are licensed)
    • Professional schools attended
    • Personal Identifiers such as NPI, DEA, FEIN, or UPIN
  6. Add up to two email addresses to receive notifications of any new activity on your account. You can also add a mobile phone number and choose to receive text message notifications.
  7. Review your entries to be sure they are correct and select Submit.

Once you complete your profile, we'll search the NPDB for reports that match the information you provide and display the results for free. You can also view and respond to any matched reports at no cost. Subscribe for $3.00 per year to access additional services such as the certified Self-Query response PDF and report disclosure history.
