What is a Self-Query?
A Self-Query generates a certified PDF response file listing any reports within the NPDB that match the profile you created. The response details the reports found or states that no matching reports were found based on your own information submitted. Certified Self-Query responses are only available through an annual $3.00 subscription.
Create a personal account and complete your profile to search the NPDB for reports about you for free, even if you do not need a certified response. If there are reports that match your profile, such as medical malpractice payments, adverse licensure or privileges actions, or judgments and convictions, you can access and respond to the report(s) through your personal account.
