What should I do if my paper Self-Query never arrived?
If your mailed Self-Query did not arrive, check the delivery status by selecting the tracking number link located in the email or text notifications received, in your Self-Query order details, or entering the tracking number directly using the USPS Tracking website Exit Image

You may be able to order one courtesy replacement copy if USPS delivery shows status similar to:
- Not available for redelivery or pickup
- Being returned to sender
- Arriving to a USPS facility in Dulles VA Distribution Center
- Delivered, PO Box Chantilly, VA
You must wait eight calendar days from your Self-Query processing date to order a replacement copy.
To order a replacement paper copy:
- Sign in to your personal account.
- Select Your Orders in your personal account.
- On the Order History page, select the Order ID link for the mailed paper copy.
- On the Order Details page, select Order a replacement.
- Select the same or change the delivery address.
- For the replacement reason, select I didn't receive my original order.
- Review your order to ensure it is correct and complete.
- Select Submit to mail the replacement copy.
You can only order one courtesy replacement copy. Additional replacement copies cost $13.00 per copy. Paper copies are sent by U.S. Certified First-Class Mail. A signature is required upon delivery. Allow 10 business days for delivery. Do not open the envelope before giving it to your licensing or certifying official.
Ask your board if they will accept certified Self-Query responses. A certified Self-Query saves you time and trouble. You don't need to sign for a mailed copy or worry about accidentally opening the envelope.
