How is the paper copy of my Self-Query delivered?
For an additional fee, paper Self-Query responses are sent by the United States Postal Service (USPS) Certified First-Class Mail and require a signature upon delivery. The NPDB does not email Self-Query responses and will not send will to third parties, such as state licensing boards. Do not open the envelope before giving it to your licensing or certifying official.
