I am applying for licensure. How do I order a Self-Query?
To complete an Individual Self-Query:
- Select Sign in on the personal account page.
- Sign In with ID.me and complete identity verification.
- Activate your personal account by reviewing and accepting the terms in the Rules of Behavior and Subscriber Agreement.
- Complete profile for your personal account.
- Be ready to provide:
- State health care license information (if you are licensed)
- Professional schools attended
- Personal Identifiers such as NPI, DEA, FEIN, or UPIN
- Be ready to provide:
- Subscribe. For an annual subscription is $3.00 you can access a current certified Self-Query PDF at any time.
- View and download your certified Self-Query PDF. The PDF response file is your personal file to do with as you wish.
The NPDB does not email Self-Query responses and does not send responses to third parties, such as state licensing boards.
