Get Your NPDB Self-Query
Health care professionals can find out if there is a report about them in the National Practitioner Data Bank (NPDB) by ordering a Self-Query.
Start
- Go to www.npdb.hrsa.gov/SQ
- Complete the Self-Query form with your identifying information.
- Enter payment information to proceed with your order.
- Verify your identity:
- Answer questions online that only you will know.
Or - Get a paper form notarized, then scan and upload it, or mail the pages to us.
- Answer questions online that only you will know.
- Get your online results in minutes.
The results show any reports found or if no reports were found.
Note: Do not open your envelope if you are sending your results to an organization. Most require the NPDB envelope to be sealed and unopened.
Finish
Your Self-Query response is only available for 45 days from the process date. You will need to submit a new Self-Query order each time a response is needed.
