How do I deactivate my personal account?
If you deactivate your account, you will no longer have online access to your information in the NPDB, including your reports and orders. Deactivating your account does not remove your information from the NPDB. If new or updated information is received about you, notifications will be sent by mail.
To deactivate your personal account:
- Select Sign In on the top right of the NPDB home page, then select Personal Account.
- On the Self-Query/View Your Report Sign In page, select Sign in.
- Sign in to your personal account with your ID.me email and password.
- Select Your Account in your personal account.
- Select the Other Functions tab.
- Select Deactivate my account.
- Select or enter a deactivation reason.
- Select Submit.
- Select Exit on the Account Deactivation Confirmation page.
To restore online access, sign in with your ID.me credentials.
