How do I view the order history in my personal account?

To view your order history:

  1. Select Sign In on the top right of the NPDB home page, then select Personal Account.
  2. On the Self-Query/View Your Report Sign In page, select Sign in.
  3. Sign in using your ID.me email and password.
  4. Select Your Orders to view your Order History.

Order History

On the Order History page, you can:

  • View a list of your orders
  • Check the status of each order
  • Select an Order ID to view Order Details

Order Details

On the Order Details page, you can:

  • Download the response included in a mailed copy
  • View your charge receipt
  • Track delivery with a USPS tracking number (if available)
  • View replacement order information (if applicable)

Access to Past Orders

  • For expired Self-Query orders, only the charge receipt is available.
  • Orders are archived after 3 years, and the charge receipt is no longer available.
