What is an NPDB report?

An NPDB report records either a medical malpractice payment paid on behalf of a health care practitioner or an adverse action taken against a health care practitioner, organization, provider, or supplier. Only certain kinds of actions may be reported to the NPDB. The NPDB collects information and maintains reports on the following:

  • Medical malpractice payments (made on behalf of practitioners)
  • Federal and state licensure certification actions
  • Adverse clinical privileges actions
  • Adverse professional society membership actions
  • Negative actions or findings by private accreditation organizations and peer review organizations
  • Health care-related criminal convictions and civil judgements
  • Exclusions from Medicare/Medicaid or other government health care programs
  • Other adjudicated actions or decisions

The reports collected apply to health care practitioners, health care entities, providers and suppliers based on the laws and regulations that govern the National Practitioner Data Bank (NPDB).

Health care practitioners and organizations may request their own report information from the NPDB at any time by ordering a Self-Query.

Note: You cannot order a Self-Query for someone other than yourself, or on behalf of an organization other than the one you represent.
