What do I do if the name on my practitioner's Self-Query doesn't match the name I have on file?
The name that appears on an individual's Self-Query is supplied by their verified ID.me account. In some cases, the name used for ID.me identity verification may differ slightly from the name on file with your state board (for example, your state board has "Jane Smith" on file, but the Self-Query states "Jane Smith Doe"). Individuals can enter other names they use in the "Additional name" drop down menu under the Personal Information section of their Self-Query request. Individuals can also update the name on their ID.me account Exit Image.
To ensure state boards obtain accurate and comprehensive data, we encourage them to query the NPDB directly.
Note: Individuals cannot submit a Self-Query on someone other than themselves.
