How long is my Self-Query response available?
If you have an active subscription for your personal account, get your current certified Self-Query response at any time. Go to Your Subscription in your personal account to access your PDF.
If you ordered a mailed paper copy with your subscription, you can view a copy of the response that was sent in the envelope. Go to the order details and select "View this response."
After 8 days, if you have not received a mailed paper copy or opened the envelope by mistake, you can order a one-time courtesy replacement.
For a Self-Query order completed before May 8, 2026, the one-time response is only available online for 45 days. After that date, the order status is changed to "Expired" and you can only view the receipt.
