I opened my mailed Self-Query order by mistake; what should I do?
If you previously ordered a paper copy, you must wait eight calendar days from your Self-Query processing date to order a courtesy replacement copy. A replacement copy can be sent to the same home or work address, or you can change the address if needed. The NPDB will not mail your Self-Query response to a third party, such as a state licensing board. Mailed paper copies can only be sent to a U.S. address, including P.O. Boxes, U.S. territories, and military addresses.
To order a replacement paper copy:
- Select Your Orders in your personal account.
- On Your Orders page, select the View Your Order History button.
- On the Order History page, select the Order ID link for the mailed paper copy.
- On the Order Details page, select Order a replacement.
- Select the same or change the delivery address.
- For the replacement reason, select I opened the sealed envelope by mistake.
- Review your order to ensure it is correct and complete.
- Select Submit to mail the replacement copy.
Paper copies are sent by U.S. Certified First-Class Mail. A signature is required upon delivery. Allow 10 business days for delivery. Do not open the envelope before giving it to your licensing or certifying official.
Ask your board if they will accept certified Self-Query responses. A certified Self-Query saves you time and trouble and eliminates the need for a signature upon delivery.
