What do I do with a letter addressed to a deceased practitioner?
If you received a notification letter for a practitioner who is deceased please do the following:
- Mail the notification letter along with a copy of the death certificate to the NPDB. Please do not email the death certificate. You can find the NPDB mailing addresses on the Contact Us page.
- If you do not have a copy of the death certificate, send the notification letter back to NPDB. Write "Return to Sender" clearly on the notification letter.
- If the letter is unopened, place the marked mail back in your mailbox or in a local mailbox for USPS collection.
- If the letter is opened, reseal the letter and mail it to the NPDB. You can find the NPDB mailing addresses on the Contact Us page.
