How do I reactivate my account after 60 days?
To reactivate an inactive account, simply sign in to the NPDB as normal. If your account needs to be reactivated, the next time you try to sign in to the NPDB, we will send you an email containing a one-time code. You will use this code to successfully reactivate your account and sign in to the NPDB.
For IQRS accounts, the NPDB will send an email 10 days in advance of your account needing reactivation.
